SSI makes monthly payments to people who have low income and few resources and are:
Age 65 or older;
Blind; or
Disabled.
If you are applying for SSI, you can complete a large part of your application by visiting our website at www.socialsecurity.gov. You also can call us toll-free at (855) 287-4792 to ask for an appointment with a Social Security representative.
Parents or guardians usually can apply for blind or disabled children under age 18. In some cases, other third parties can apply for children.
You should bring certain items when you apply. Even if you do not have all of the things listed below, apply anyway. The people in the Social Security office can help you get whatever is needed.
Please bring:
- Your Social Security card or a record of your Social Security number;
- Your birth certificate or other proof of your age;
- Information about the home where you live, such as your mortgage or your lease and landlord's name;
- Payroll slips, bank books, insurance policies, burial fund records and other information about your income and the things you own;
- The names, addresses and telephone numbers of doctors, hospitals and clinics that you have been to, if you are applying for SSI because you are disabled or blind;
- Proof of U.S. citizenship or eligible noncitizen status; and
- Your checkbook or other papers that show your bank, credit union or savings and loan account number. If you are approved for SSI, you must receive your

I had several questions about suspending my benefits, and Gloria was extremely knowledgeable, helpful, and personable. For those questions she could not answer immediately, she checked with a co- worker and searched their data bank. She even called me back with some additional info that she found later. She is exceptional!
I have called this office many times over the past year and each time I have talked with extremely helpful, polite and patient employees. And I have gotten a different person each time. Good morning, What documents do I need to request a new social security card for a minor child? Can one parent request the document or both parents need to be present?
Called a few weeks ago to schedule an appointment for my mother who is 76 years old but still works full time and needs to change her benefits. I was told last week that all I needed was her social security number to make the appointment. I call today and spoke with this very rude jerk who told me his name was Jason and he stated she had to call herself and I couldn't make the appointment for her. When I asked to speak with a supervisor he refused and told me to call my congressman and hung up.

The Security Officer that works at this location is extremely rude/ugly/dis-respectful and treats you like you are a criminal and/or doing something wrong just by driving in the parking lot. Once I find out how to contact someone about his behavior and how he has treated me on 3 to 4 different occasions as well as another co-worker I will do so.
