SSI makes monthly payments to people who have low income and few resources and are:
Age 65 or older;
Blind; or
Disabled.
If you are applying for SSI, you can complete a large part of your application by visiting our website at www.socialsecurity.gov. You also can call us toll-free at 866-964-7583 to ask for an appointment with a Social Security representative.
Parents or guardians usually can apply for blind or disabled children under age 18. In some cases, other third parties can apply for children.
You should bring certain items when you apply. Even if you do not have all of the things listed below, apply anyway. The people in the Social Security office can help you get whatever is needed.
Please bring:
- Your Social Security card or a record of your Social Security number;
- Your birth certificate or other proof of your age;
- Information about the home where you live, such as your mortgage or your lease and landlord's name;
- Payroll slips, bank books, insurance policies, burial fund records and other information about your income and the things you own;
- The names, addresses and telephone numbers of doctors, hospitals and clinics that you have been to, if you are applying for SSI because you are disabled or blind;
- Proof of U.S. citizenship or eligible noncitizen status; and
- Your checkbook or other papers that show your bank, credit union or savings and loan account number. If you are approved for SSI, you must receive your

Thanks much, Clay, for the fast and excellent service when I called in today to see if my Medicare Part B application has been received. You activated it effective today and it took about 3 minutes to do that. This is the second time I've called the Des Moines Social Security office and received better than expected service.
Government workers. Worst customer service ever. Phone line hangs up, cant speak to anyone. Its like pulling teeth just getting a basic update completed. Day 4 and still unable to contact anyone. Office is closed... so, I guess I'm just out of luck.

I was contacted by phone of someone telling me my ss number was being used in Texas by criminals shipping narcotics and money. I was fooled for a while but when they told me to go to Target that I hung up. He gave me a name, badge no., And case no. I want to report him but couldn't get thru to the Iowa Attorney general. I called the Des Moines SS Office on Thursday, July 23 at 11:05 concerning fraudulent phone calls and was put in touch with a representative that could NOT have been more rude! I am now aware that this happens a lot but this is the first time for me. She talked over me constantly when I was just trying to ask a question. Her tone was extremely condescending. The first comment she made before I could even ask my question was to just change my phone number and that they (SS office) could do nothing. I was TRYING to ask if they had any red flags associated with my number but was unable to because of her constantly interrupting me with very rude comments. When I asked for her name she hung up. . . .REALLY?!!! This is who you have on the phone to help people?
