SSI makes monthly payments to people who have low income and few resources and are:
Age 65 or older;
Blind; or
Disabled.
If you are applying for SSI, you can complete a large part of your application by visiting our website at www.socialsecurity.gov. You also can call us toll-free at 888-371-0791 to ask for an appointment with a Social Security representative.
Parents or guardians usually can apply for blind or disabled children under age 18. In some cases, other third parties can apply for children.
You should bring certain items when you apply. Even if you do not have all of the things listed below, apply anyway. The people in the Social Security office can help you get whatever is needed.
Please bring:
- Your Social Security card or a record of your Social Security number;
- Your birth certificate or other proof of your age;
- Information about the home where you live, such as your mortgage or your lease and landlord's name;
- Payroll slips, bank books, insurance policies, burial fund records and other information about your income and the things you own;
- The names, addresses and telephone numbers of doctors, hospitals and clinics that you have been to, if you are applying for SSI because you are disabled or blind;
- Proof of U.S. citizenship or eligible noncitizen status; and
- Your checkbook or other papers that show your bank, credit union or savings and loan account number. If you are approved for SSI, you must receive your

Opening Up a MySocialSecurity Account

I’m a ward of the state and I need a new Social security card. I lost It, I do have a copy of my birth certificate, I know my SS number, I do have a police report of my lost documents. How do I obtain a new SS card? I’m 19, and I don’t have a valid ID.
I am leaving this message to leave a remark about the staff member with the last name Rivas in Evanston, IL. Your webpage states to call the office to make an appointment. I did so and spoke to Rivas. Rivas did not let me finish what I had to say and interrupted by saying mail in your requests for any replacement....etc. I asked her to slow down because she interrupted me. I got another rude response from Rivas that replacement, change of names are to be mailed. Am I supposed to mail in my Original Driver's license as well? I remain calm and professional when dealing with the public. I get that Rivas keeps repeating the same thing over about the application to other customers but Rivas does not have to be rude. I mentioned to Rivas to be more courteous and respectful. I then asked for the supervisor name (Jacobo) and disconnected the call. YOUR WEBSITE SHOWS that I have to show my original documents. How will I present them? It is very confusing on the type of services you are taking by appointment and what forms you want via US mail. You can check your phone records and google reviews about how rude Rivas is. The supervisor's name is 'Jacobo.' Have a better day.
Good morning, I’m not the type of person to write in about a bad experience, but I was just for yelled at aggressively by a Mr. Marr and was treated unbelievably disrespectfully. I understand we are all stressed but I felt he wasn’t listening to me and demanding information from me that I already had explained I understood. Finally he screamed at me and said I’VE ASKED YOU 3 times, answer or I’ll hang up. I listened to what he said but I was fighting back tears and I had to hang up because I was too upset to ask anymore questions. It ruined an already stressful day. Like I said, I know everyone has bad days but no one should be treated with such disrespect and hostility. I also didn’t get my questions answered. Maybe I caught him on a bad day but it rattled me and left a bad taste in my mouth about the service at SSA. If I don’t hear back I will post a Yelp and Twitter review because this was just unjustified.
I need to replace my social security card asap. If I mail in a request, I have to mail in my driver's license, and I don't want to do that. Can I do this process in person at your office?
