SSI makes monthly payments to people who have low income and few resources and are:
Age 65 or older;
Blind; or
Disabled.
If you are applying for SSI, you can complete a large part of your application by visiting our website at www.socialsecurity.gov. You also can call us toll-free at 877-335-4114 to ask for an appointment with a Social Security representative.
Parents or guardians usually can apply for blind or disabled children under age 18. In some cases, other third parties can apply for children.
You should bring certain items when you apply. Even if you do not have all of the things listed below, apply anyway. The people in the Social Security office can help you get whatever is needed.
Please bring:
- Your Social Security card or a record of your Social Security number;
- Your birth certificate or other proof of your age;
- Information about the home where you live, such as your mortgage or your lease and landlord's name;
- Payroll slips, bank books, insurance policies, burial fund records and other information about your income and the things you own;
- The names, addresses and telephone numbers of doctors, hospitals and clinics that you have been to, if you are applying for SSI because you are disabled or blind;
- Proof of U.S. citizenship or eligible noncitizen status; and
- Your checkbook or other papers that show your bank, credit union or savings and loan account number. If you are approved for SSI, you must receive your

I called to get my last name changed on my Social security card. The guy I spoke with said I have to send in original documents such as my driver’s license and said it could take up to 30 days. I said I cannot do that as I would not have a license to drive around. The guy said that I do not have to change my name and that some people don't do it for up to 10 years. I have already changed it on my marriage certificate and my driver's license. It is also required for me to do so at my work so that my W2 will use the correct name for taxes so it is quite important. I will try to set up an appointment, but would have been more appreciative if the gentlemen offered it to me while I was on the phone for a half hour. He told me all offices were closed and unsure when they would re-open due to COVID even though ND is almost fully functioning/open again, which I understand, but an appointment could have been made.
I’m trying to get my last name changed on my Social security card for over two weeks now . The last I just talked to said that I have to send in original documents such as my ID or driver’s license . So I specifically asked her do you want me to send in my drivers license and then drive around illegally while you have it.
