SSI makes monthly payments to people who have low income and few resources and are:
Age 65 or older;
Blind; or
Disabled.
If you are applying for SSI, you can complete a large part of your application by visiting our website at www.socialsecurity.gov. You also can call us toll-free at 866-613-3962 to ask for an appointment with a Social Security representative.
Parents or guardians usually can apply for blind or disabled children under age 18. In some cases, other third parties can apply for children.
You should bring certain items when you apply. Even if you do not have all of the things listed below, apply anyway. The people in the Social Security office can help you get whatever is needed.
Please bring:
- Your Social Security card or a record of your Social Security number;
- Your birth certificate or other proof of your age;
- Information about the home where you live, such as your mortgage or your lease and landlord's name;
- Payroll slips, bank books, insurance policies, burial fund records and other information about your income and the things you own;
- The names, addresses and telephone numbers of doctors, hospitals and clinics that you have been to, if you are applying for SSI because you are disabled or blind;
- Proof of U.S. citizenship or eligible noncitizen status; and
- Your checkbook or other papers that show your bank, credit union or savings and loan account number. If you are approved for SSI, you must receive your

Hi, if I want to do an appointment to change the status of my sister´s social security and get one because I lost it, where I can get that?
I have tried calling to make an appointment and no one answer. I have been on hold for almost an hour and nothing. I cannot even leave a message. Why can we only make an appointment through a number that is not being answered?
I called the office to find out what the process was to change my name on the SS card from maiden to married. I was told the process was to mail in my original Marriage certificate and or current Passport or current Driver's license. I was told the total process would take 4 to 6 weeks. I did the math as I was moving out of the country and submitted my passport with my marriage certificate. I did receive my new SS card but never got my documents back. Two weeks ago I got through to a Customer Service rep who said she would put in a request to her supervisor on my behalf and I would be contacted. If they find the documents were sent they could give me a tracking number. It has now been 2 1/2 weeks since that one contact and 9 1/2 weeks since they processed my SS card. have heard back from anyone, I have left messages with no return call. Each time I call I am told only to be told nothing more can be done. Today I called and was put on hold for a supervisor and I've been on hold for 2 hours and 15 minutes. It's now 6 pm so I guess II was put on hold and then everyone went home. I'm in tears as in 3 weeks I am out of my house with a home rented in Mexico and no way to get there. I just want to cry.
This office is poorly run. I was instructed by a representative at the 1 (800) 772-1213 to complete SS replaced card form to correct my date of birth with SS. I was also instructed to mail an original identification with my form to my local office 12249 Pembroke Rd, Pembroke Pines, FL 33025. I sent my passport along with a completed replacement card form, over 3 weeks ago via certified mail. Last week I received a replacement card and not my passport. I then called the 1-800-772-1213 # to find out when I could expect my passport back. I was informed by the associate that my date of birth had not been changed and that I needed to call my local office. I have been calling the local office 866-613-9362 and 866-643-3406 to no avail. The phone system is asking me for a representative's extension name. I do not have either so i am unable to go beyond that point. All i want to know is why my information has not been update and when i can expect to get my passport back.
I want to recognize Patrick Dibs for excellent service. I am retiring this week and contacted your office for information and to see if Medicare forms I had mailed had arrived and been processed so I wouldn't have to be on Cobra longer than necessary. Mr. Dibs looked to see if my forms had been processed and, when he saw they hadn't been, he asked me to fax them to his attention. He called me when he received them to advise me of when I would be officially on Medicare Part B and was so helpful that I directed my husband's forms to his attention, as well. This was only weeks ago and we both received information confirming our enrollment. You often hear negative comments about working with the Federal government but Mr. Dibs proved them wrong. Many thanks to him!
I need an appointment for a first time social security card. I filled out the e-records online, and I have all documents needed in an envelope. I recently received a permanent resident card as well.
