SSI makes monthly payments to people who have low income and few resources and are:
Age 65 or older;
Blind; or
Disabled.
If you are applying for SSI, you can complete a large part of your application by visiting our website at www.socialsecurity.gov. You also can call us toll-free at (877) 316-4418 to ask for an appointment with a Social Security representative.
Parents or guardians usually can apply for blind or disabled children under age 18. In some cases, other third parties can apply for children.
You should bring certain items when you apply. Even if you do not have all of the things listed below, apply anyway. The people in the Social Security office can help you get whatever is needed.
Please bring:
- Your Social Security card or a record of your Social Security number;
- Your birth certificate or other proof of your age;
- Information about the home where you live, such as your mortgage or your lease and landlord's name;
- Payroll slips, bank books, insurance policies, burial fund records and other information about your income and the things you own;
- The names, addresses and telephone numbers of doctors, hospitals and clinics that you have been to, if you are applying for SSI because you are disabled or blind;
- Proof of U.S. citizenship or eligible noncitizen status; and
- Your checkbook or other papers that show your bank, credit union or savings and loan account number. If you are approved for SSI, you must receive your

Benefits suspended because of their mistakes and I provided all documentation even had to paid doctor office to fill out paperwork because of their mistakes. They are costing me money that I don’t have in late payments, because now my bills are late.
My benefits were suspended due to internal mistake and I’ve provided the documents to show that they should not have been suspended and yet again the documents were misread and my benefits remain suspended. I have called everyday only to be told is busy and will call me back, we are at business day 7. I call everyday but I’ve yet to get a call back. All because of an internal mistake followed by another mistake on document interpretation and lack of communication from this office. I have no other income and they have left me without 4 medications I need to be taking. All this could be avoided if a simple phone call could be returned. I understand they are busy but if the situation were reversed and they had no medicine , oxygen, food , money for fuel and was about to lose life insurance policy that has been had for 5 years. It’s one thing to make a mistake that’s human it’s another to be so arrogant that you cannot look at paperwork closely and own and fix mistake to help someone out and return one of my daily calls that each day I’m told will happen. So my review of this office is very poor, however, I would change if they would simply do what they say each day or at least one of them.
I need to make a appointment for assistance filling out my my disability review report.I have tried my best, but am terrified to mess it up!!! I just completed the entire review process in 2018? I think I cant be for sure.
The lady who works there who answers the phone is unpleasant, sarcastic and the most unprofessional person I have ever spoken to. Every time I call she gives me the run around and sounds like she hates her life. She is not helpful at all and refuses to help. The system is flawed and would be much better if someone more helpful and polite would be in that position.
I have been trying to get my social security card so I can start work but you guys are not helping me out at all.
