SSI makes monthly payments to people who have low income and few resources and are:
Age 65 or older;
Blind; or
Disabled.
If you are applying for SSI, you can complete a large part of your application by visiting our website at www.socialsecurity.gov. You also can call us toll-free at (866) 964-5054 to ask for an appointment with a Social Security representative.
Parents or guardians usually can apply for blind or disabled children under age 18. In some cases, other third parties can apply for children.
You should bring certain items when you apply. Even if you do not have all of the things listed below, apply anyway. The people in the Social Security office can help you get whatever is needed.
Please bring:
- Your Social Security card or a record of your Social Security number;
- Your birth certificate or other proof of your age;
- Information about the home where you live, such as your mortgage or your lease and landlord's name;
- Payroll slips, bank books, insurance policies, burial fund records and other information about your income and the things you own;

Hayward office helped me even when the office had appts only. Open for appts but they were able to help me change bank information and within 2 weeks my new bank contacted me with news from Social security. Thankyou.

Today I called the nationwide SSA number and could not get through at all, not even a recording telling me how long it would be for someone to answer the phone. I waited for 24 minutes. Then I called the Hayward office at and after 3 minutes waiting I was disconnected. I applied and I have heard nothing as of today.
Customer service from Hayward Office of social Security Is Very Unproductive. The phone etiquette you have with customers On the phone is very Poor. I’m having Strong Conflicting issues. People from this office do not return calls. You customer service reps. Very Rude that Entire Office Staff needs to be RETRAINED ON COMPANY POLICIES AND WORK PROCEDURES. If you identify a customer on the phone with five pieces of information, you should be able to obtain information on yourself. Instead, Your told I can’t give you that information. Then what’s the point of verifying a person if you can’t give them information about themselves. ALSO THAT SS5 Form is a joke. I filled out this form mailed in birth certificate, Drivers License. Filled out their Form. I wrote a letter enclosed with other documents. Sent Certified Mail. Letter was received the person whom opened the letter NEVER READ ANYTHING REGARDING THE PURPOSE Of THE CERTIFIED LETTER documents. I simply wanted my First name added back onto my new social security card, for what ever reason this was Not Comprehended by Office Staff this person just sent out replace card saying exactly what my old card had stated. I went through this process twice and Received the same Results. Does anyone know what Customer Service is anymore. Now I’m getting the run around because someone failed to do their job in a correct and ethical manner. Whom ever the person may be that some that. They Need to be Retrained or Terminated from Position because it’s apparent their just there for a paycheck. This is Totally Sad. I curious to hear the Response about this one again. Gone through this Twice. Over the same issue, no one knows what to do. I wish they would relieve all those from the Hayward office of the Management, Supervisiors, Customer Service Trps.
