SSI makes monthly payments to people who have low income and few resources and are:
Age 65 or older;
Blind; or
Disabled.
If you are applying for SSI, you can complete a large part of your application by visiting our website at www.socialsecurity.gov. You also can call us toll-free at (866) 592-0802 to ask for an appointment with a Social Security representative.
Parents or guardians usually can apply for blind or disabled children under age 18. In some cases, other third parties can apply for children.
You should bring certain items when you apply. Even if you do not have all of the things listed below, apply anyway. The people in the Social Security office can help you get whatever is needed.
Please bring:
- Your Social Security card or a record of your Social Security number;
- Your birth certificate or other proof of your age;
- Information about the home where you live, such as your mortgage or your lease and landlord's name;
- Payroll slips, bank books, insurance policies, burial fund records and other information about your income and the things you own;

This is very frustrating.. I need my SS Card replaced asap and filled out the form SS-5. Provided them the SS5 form as requested and drop this off to the Jamaica office back in April and now Parent's name were not verified. it's been 15 months.. if all the restaurant are at 100% capacity why cant the SS Office at least have someone onsite. I cannot even setup a schedule online? I will need to call the number for the local office. At least NY DMV, you can schedule online and have a check list of what's needed.

I have called the office a few times and the representatives are rude and hang up or disconnect the line. Some don't even respond when you get through, they just automatically disconnect the line. No one is overseeing them as they are working from home now.

I just call this location for an inquiry about my name change case that have been going on for 10 years now. First I received my personal documents with someone else name on it.
