SSI makes monthly payments to people who have low income and few resources and are:
Age 65 or older;
Blind; or
Disabled.
If you are applying for SSI, you can complete a large part of your application by visiting our website at www.socialsecurity.gov. You also can call us toll-free at (877) 405-5870 to ask for an appointment with a Social Security representative.
Parents or guardians usually can apply for blind or disabled children under age 18. In some cases, other third parties can apply for children.
You should bring certain items when you apply. Even if you do not have all of the things listed below, apply anyway. The people in the Social Security office can help you get whatever is needed.
Please bring:
- Your Social Security card or a record of your Social Security number;
- Your birth certificate or other proof of your age;
- Information about the home where you live, such as your mortgage or your lease and landlord's name;
- Payroll slips, bank books, insurance policies, burial fund records and other information about your income and the things you own;
- The names, addresses and telephone numbers of doctors, hospitals and clinics that you have been to, if you are applying for SSI because you are disabled or blind;
- Proof of U.S. citizenship or eligible noncitizen status; and
- Your checkbook or other papers that show your bank, credit union or savings and loan account number. If you are approved for SSI, you must receive your

My SSDI application was denied. I want to appeal the decision. I would like to have a fair hearing to explain my disability which is permanently disabled since two back surgeries in 2018. There is more documents to present to the court regarding mtg health. I suffer with depression and have documents from past.
Horrible location apparently can’t do anything right. Can’t respond to voicemails, return calls, answer calls, crap communication and customer service. Don’t know what in gods name they’re doing. Back in December I had filed my application to receive my Social Security. A month went by, I followed up and called the 800 number. They said ther was a backup and to wait. I waited another couple of weeks, called and was told there was a backup, and that maybe I should go to my local office. Last Friday I took a ride with one of my co-workers to the office and found out it wasn't the local office where my claim was. I did have the pleasure of meeting Donna Jani. It was almost closing time, yet she took the time to review my claim, have it transferred to her office and completed it while I was there. She told me I would get my payment on the third Wednesday of the month. I told her that was 5 days away with a holiday in between. Then we had the bad weather. I checked my status, still not approved. This morning I went to check my bank account and there was a nice deposit from SSA.Went back and checked my application. It was approved. Donna, thank you so much for what you did. I hope your management reads this and recognizes what a terrifiic employee and person in their office. Again, thank you !!!!!!!!! Roger Leahy
