SSI makes monthly payments to people who have low income and few resources and are:
Age 65 or older;
Blind; or
Disabled.
If you are applying for SSI, you can complete a large part of your application by visiting our website at www.socialsecurity.gov. You also can call us toll-free at 877-692-3143 to ask for an appointment with a Social Security representative.
Parents or guardians usually can apply for blind or disabled children under age 18. In some cases, other third parties can apply for children.
You should bring certain items when you apply. Even if you do not have all of the things listed below, apply anyway. The people in the Social Security office can help you get whatever is needed.
Please bring:
- Your Social Security card or a record of your Social Security number;
- Your birth certificate or other proof of your age;
- Information about the home where you live, such as your mortgage or your lease and landlord's name;
- Payroll slips, bank books, insurance policies, burial fund records and other information about your income and the things you own;
- The names, addresses and telephone numbers of doctors, hospitals and clinics that you have been to, if you are applying for SSI because you are disabled or blind;
- Proof of U.S. citizenship or eligible noncitizen status; and
- Your checkbook or other papers that show your bank, credit union or savings and loan account number. If you are approved for SSI, you must receive your

Opening Up a MySocialSecurity Account

Monthly cash benefits are paid to the eligible individual with a disability and his or her eligible dependents throughout the period of disability.
SSI
SSI benefits also are payable to people 65 and older without disabilities who meet the financial limits. People who have worked long enough may also be able to receive Social Security disability or retirement benefits as well as SSI.
We stopped in to the Social Security Office in Painesville Ohio yesterday Wed March 15th, 2023. We arrived at 9:15 am second person in the waiting area. The representative called my number in less than 12 minutes my issue was taken care of. The representative was knowledgeable and was able to take care of my issue. Very nice respectful, helpful, knowledgeable. Thank you for your time and help. I truly appreciated it.
Applied for replacement of social security card. I am not sure about sending my drivers license or birth certificate in the mail. Am I able to drop these items off to have the submission completed then and there or will I still have to wait up to 14 days to get my documents back?
I spoke with Mr. Zell in the Painesville, Ohio office about my part B Medicare and he was very polite and helpful, went above and beyond what I asked (or even knew that I needed). His help was very much appreciated and he should be cloned!
I just got off the phone with a Mrs. P the Painesville OH location. She was not very helpful. I tried to ask questions but she was very short and rude. I tried to explain how I want to make sure I am doing everything correctly as I am a rep payee for my son. I am very disappointed in the service I received today.
I sent a request for a new SS card because I lost mine. I included a copy of life insurance form and medicare card and medical card for ID. I have not heard back.
Considering the office is only open for appointments, it would be beneficial if a drop box was available for paperwork. Because of the insanity of the postal system, I just received paperwork for my disabled son that is due in 2 days that was mailed to me 4 weeks ago. Accomodations need to be made to meet the needs of the people during this time.
