SSI makes monthly payments to people who have low income and few resources and are:
Age 65 or older;
Blind; or
Disabled.
If you are applying for SSI, you can complete a large part of your application by visiting our website at www.socialsecurity.gov. You also can call us toll-free at (877) 897-0603 to ask for an appointment with a Social Security representative.
Parents or guardians usually can apply for blind or disabled children under age 18. In some cases, other third parties can apply for children.
You should bring certain items when you apply. Even if you do not have all of the things listed below, apply anyway. The people in the Social Security office can help you get whatever is needed.
Please bring:
- Your Social Security card or a record of your Social Security number;
- Your birth certificate or other proof of your age;
- Information about the home where you live, such as your mortgage or your lease and landlord's name;
- Payroll slips, bank books, insurance policies, burial fund records and other information about your income and the things you own;
- The names, addresses and telephone numbers of doctors, hospitals and clinics that you have been to, if you are applying for SSI because you are disabled or blind;
- Proof of U.S. citizenship or eligible noncitizen status; and
- Your checkbook or other papers that show your bank, credit union or savings and loan account number. If you are approved for SSI, you must receive your

Hello I am trying to schedule an appointment to get a replacement social security card. I have tried to call to schedule on but on hold for 20 minutes.
I called Social security office in Pampa to request application be sent to me at my address in Borger. I need to change name and get new card. It has been over 3 weeks and I never received application. I call almost daily to find out if they could send another or if ever sent first one. It is impossible to talk to anyone. Spent hours on phone. Most other agencies are open for business.

I am VERY disappointed in Pampa SS office. While in the process of trying to figure out who stole my Moms identity and while we waited for Someone from SS Pampa to return our repeated calls. My Dad and I finally decided no one was going to call us back so we made another trip to the Pampa office to find out what they found out. On our way back home my Mom passed away. Then today we called to change the banking information and the lady (Sandra) called us back and told us to call back. Which we did and after almost an hour on hold we were told my father was not Eligable for the $255 payment. REALLy!!!! That is why we were on hold for almost 1 hour to tell us this. Why didn't Sandra leave that little bit of information on the message she left for my Dad??? Not A Happy Person!!!! Our time means something too!!!!!!
