SSI makes monthly payments to people who have low income and few resources and are:
Age 65 or older;
Blind; or
Disabled.
If you are applying for SSI, you can complete a large part of your application by visiting our website at www.socialsecurity.gov. You also can call us toll-free at 877-405-0488 to ask for an appointment with a Social Security representative.
Parents or guardians usually can apply for blind or disabled children under age 18. In some cases, other third parties can apply for children.
You should bring certain items when you apply. Even if you do not have all of the things listed below, apply anyway. The people in the Social Security office can help you get whatever is needed.
Please bring:
- Your Social Security card or a record of your Social Security number;
- Your birth certificate or other proof of your age;
- Information about the home where you live, such as your mortgage or your lease and landlord's name;
- Payroll slips, bank books, insurance policies, burial fund records and other information about your income and the things you own;
- The names, addresses and telephone numbers of doctors, hospitals and clinics that you have been to, if you are applying for SSI because you are disabled or blind;
- Proof of U.S. citizenship or eligible noncitizen status; and
- Your checkbook or other papers that show your bank, credit union or savings and loan account number. If you are approved for SSI, you must receive your

SSI
SSI benefits also are payable to people 65 and older without disabilities who meet the financial limits. People who have worked long enough may also be able to receive Social Security disability or retirement benefits as well as SSI.
After speaking with Angel of the Willimantic Social Security Office, I forwarded payment in the amount of $34.00 to receive a copy of a vision evaluation that was completed by Dr. Angel directed me to mark the request 'urgent' and he promised to send me both an electronic copy and a hard copy of this report upon receipt of this request and the payment. Despite multiple phone calls and messages left - for both and his supervisor, Mr. John - all that I have received is written confirmation that the check was processed. An employee named Sean was able to verbally confirm for me that he had processed this payment and that Mr. Correa had received the request. However, nothing has been delivered, to date, and not responded to my phone calls nor has anyone confirmed the date of the mailing of this confidential document. I have also provided the correct email address in writing and over the phone multiple times but still nothing. I do believe that a government agency cashing a check without providing what has been paid for constitutes fraud and I would to like to file a formal complaint.

I would truly like to thank Mrs Kelly who works at your Willimantic Social Security Office. I was there on Saturday, June 27, 2018 (11:30 am) to file information with Social Security. to whom it may concern i've had a appointment 3/2018 and the gentlemen i spoke with said he would be able to get all my documentation from the doctors. He also said it would only take 30 days before i get my disability benefits and which i have not received yet ? i already my appointment for disability.. i'm also homeless and living out in a tent i was staying at the no freeze ion Willimantic because tha'ts the only shelter at that time that had room.. but now i have to to get this tumor out which is decaying my left temporal, i need surgery asap !! i can not do it living in a tent because no freeze closes there shelter in april so we have to tent.. i can't stat in a shelter do to the germs and bacteria that roam around the place specially having brain surgery...
